Provides community services for individuals with developmental disabilities and their families including family support assistance, group homes, sheltered workshops, community intergrated employment, guardianship assistance, assisted living, and a voucher respite repayment program.

ELIGIBILITY
Must be 3 years of age or older, have a primary diagnosis of intellectual disabilities, and meet Medicaid Long Term Care criteria.
DOCUMENTS REQUIRED
Must provide birth certificate, school records that document the intellectual disability presented prior to 18 years of age, financial records, Social Security and Medicaid benefit information.
