Provides assistance for people to remain in their own home for as long as possible by matching an individual's needs with community resources. Based on assessment, the case manager works with the individual and the family to identify areas where assistance is needed and then develops and coordinates an individualized plan that provides information and referral, linkage to resources and services, monitoring and follow-up to fill the gaps that threaten independence and safety. Also provides assistance with applications for other programs, e.g., the ADvantage program.

Offers a managed care model of acute and long-term care that integrates the provision and financing of medical and long-term care services. The PACE model is centered on the belief it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever safely possible.
