Provides a trauma-informed medical home to children living in an out-of-home placement or with their biological family, who need continuity in health screenings, diagnoses and trauma-informed treatment.
SERVICE HOURS
Mon-Fri 8am-5pm
INTAKE PROCEDURE
Professional referral required.
FEES
Accepts Medicaid/SoonerCare (including help in applying for Medicaid), Insurance; financial assistance available for initial screenings.
ELIGIBILITY
Must be 18 years of age or younger.
