SERVICE DESCRIPTION
Provides home-based mental health wraparound services to severe emotional or behavioral disturbances that place youth at risk for out-of-home or out-of-school placement.

ELIGIBILITY
Must be member of a federally recognized tribe. Must be aged 0 to 21 years with DSM diagnosis or be diagnosable. Must be at-risk for out-of-home or out-of-school placement and involved with two or more systems. Examples: School, JSU, Mental Health, DHS, Juvenile Drug Court, Probation, or Parole.
DOCUMENTS REQUIRED
Must have CDIB card. Must be member of a federally recognized tribe. Must provide photo ID/driver's license of parent/legal guardian.
