Financial Assistance
SERVICE DESCRIPTION
Assists children who receive a Supplemental Security Income - Disability Payment to receive assistance in getting needed equipment and services. This includes adaptive equipment, specialty formula, and diapers.

INTAKE PROCEDURE
** Effective Fri (3/20/20), DHS is transitioning all office visitation to a 'by appointment only' policy. This will include directing customers first to OKBenefits.org and OKDHSLIVE.org. In the event that our customers cannot be served online, the call center is scaling up to accommodate higher needs. Any outside visitor to our county offices must have an appointment set through (405) 522-5050. During the appointment process, the call center team will work to solve for our customer needs utilizing other tools available to them first.**

ELIGIBILITY
Must reside in service area, be a child 18 years of age or younger, and already receive a Supplemental Security Income disability payment from the Social Security Administration.

SERVICE DESCRIPTION
Provides monthly cash assistance based on family's income, resources and child's deprivation (child deprived of adequate financial support because of a parent's death, continued absence from the home, physical or mental incapacity or chronic unemployment). Cash assistance is available on a time-limited basis. The purpose is to provide temporary support in meeting basic needs, training leading to employment, employment services and childcare assistance for qualified families with children.

INTAKE PROCEDURE
Application available online at www.okdhs.org by searching for Request for Benefits Form in the search bar. Completed forms must be submitted to local DHS office in person, by fax or mail.

None.
ELIGIBILITY
Must be a US citizen, reside in service area, and meet income guidelines.
DOCUMENTS REQUIRED
Must provide proof of income, proof of residence, relationship with child, age of child, and proof of citizenship.
