Mon-Thu 10am-2:30pm, Fri 9am-12 noon or by appointment
INTAKE PROCEDURE
Walk-ins accepted.

None.
ELIGIBILITY
Must reside in service area, have a child enrolled in area school, or reside in zip code 74127 and receive Sand Springs water service. Must be willing to meet at scheduled times with a mentor.
DOCUMENTS REQUIRED
Must provide photo ID/driver's license, proof of residence or student school enrollment, and proof of income.
