SERVICE HOURS
Mon-Wed 8:30am-5:30pm, Thu 8:30am-7pm Fri 8:30am-5:30pm, Sat 9am-1pm *1st Thursday of each month 12 noon-7pm
INTAKE PROCEDURE
Appointment preferred. Walk-ins are taken on a first-come, first-served basis.
FEES
Accepts Medicaid/Soonercare, Medicare, and private insurance. There is a sliding scale available for patients that are uninsured with low income.
ELIGIBILITY
No restrictions.
DOCUMENTS REQUIRED
Please provide photo identification. Insurance information required if available. Proof of income is needed to qualify for the sliding scale.
