ELIGIBILITY
Must meet income guidelines, be nutritionally at risk (pregnant and/or breastfeeding women, non-breastfeeding postpartum women, infants, and children 5 years of age or younger). Individuals currently approved for Soonercare, TANF, SNAP, or FDIPR automatically qualify.
DOCUMENTS REQUIRED
Must provide photo ID/driver's license, proof of income, proof of residence, and if pregnant, proof of pregnancy.
