SERVICE DESCRIPTION
Pays benefits to adults with disabilities and children who have limited income and cannot work because they have a medical condition that is expected to last at least one year or result in death. Federal law requires this very strict definition of disability. SSI benefits also payable to individuals 65 years of age or older without disabilities who meet the financial limits.

ELIGIBILITY
Must be 18 years of age or older; reside in the United States or one of its territories/commonwealths; and meet two different earnings tests: 1) A “recent work” test based on age at the time of disability; and 2) A “duration of work” test to show that individuals has worked long enough under Social Security. Certain blind workers have to meet only the “duration of work” test. Must have a medical condition that has prevented or is expected to prevent individual from working for at least 12 months or end in death.
DOCUMENTS REQUIRED
Must provide Social Security number; birth or baptismal certificate; names, addresses and phone numbers of the doctors, caseworkers, hospitals and clinics that provided care; names and dosage of all medication; medical records already in one's possession; laboratory and test results; a work summary; and a copy of most recent W-2 Form (Wage and Tax Statement) or, if self-employed, federal tax return for the past year.

If one of these documents in not available and cannot be replaced within 10 days, other documents may be provided including: Employee ID card, School ID card, Health insurance card (not Medicare), or U.S. military ID card. All documents must be either originals or copies certified by the issuing agency.
