None.
ELIGIBILITY
Must be enrolled in hospice program. Must be in final weeks end of life (30 days or less). Must have Do Not Resuscitate (DNR) in place or ready to sign one.
DOCUMENTS REQUIRED
Must provide medical records. Must have advance directives.
