Provides Medicaid services to help people stay at home instead of going to a nursing home. Services may include adult day health care, case management, consumer directed personal assistance services & supports, home-delivered meals, home modification, hospice, occupational therapy, personal care, physical therapy, prescription drugs, specialized equipment and supplies, speech therapy and supported restorative assistance.

Mon-Fri 8am-5pm

Application required. Applicant will be scheduled for an assessment, in which a nurse will come to the home to determine if applicant is medically eligible. A caseworker will be assigned to determine financial eligibility.

Must reside in service area, meet income guidelines, require nursing home level of care, and qualify for Medicaid. Service cannot be provided for children or those individuals with developmental disabilities.
