SERVICE DESCRIPTION
Provides nutrition and breastfeeding education, supplemental foods, and improved access to regular health care and social services to low and moderate income pregnant, postpartum and breastfeeding women, and young children with, or at risk of developing nutrition related health problems. Offers Farmer's Market program seasonally.

ELIGIBILITY
Must meet income guidelines, reside in service area, and be a woman who is pregnant, breast-feeding or recently given birth (including women who have had a miscarriage or abortion) and any child from birth to 5 years of age.
DOCUMENTS REQUIRED
Must provide proof of income, photo ID/driver's license, proof of residence, proof of pregnancy or birth (hospital release or shot record).
