SERVICE DESCRIPTION
Helps members sustain independent living and vocational skills, assisting with case management, and providing opportunities for GED preparation. Also provides integration for seeking to return to work on job placements in local businesses.

Mon-Fri 9am-5pm
INTAKE PROCEDURE
Application required.

None.
ELIGIBILITY
Must be 18 years of age or older, diagnosed with a serious mental illness, and have no history of violence. Membership application must be accepted prior to receiving services.
DOCUMENTS REQUIRED
Must complete intake application for membership. Membership application needs information from mental health provider. Must provide current COVID-19 vaccination card.
