Provides complete restorative dental care, including extractions and removable dentures or partial dentures.

INTAKE PROCEDURE
Application required. Call or visit website for application. Voicemails required for return service as staff is remote. Drop box accessible at office location for complete applications contained in a sealed envelope.
FEES
None.
ELIGIBILITY
All clients must be low income according to the federal poverty guidelines specific to Medicaid specifications and may not have any savings accounts, CD's, Trust accounts, IRA's or any other means of paying for dental care.
*Qualifications for Preventive/Restorative dental treatment: Either 65 years or older, receiving a social security administration check or a referral from a Medical Assistance Program (this qualification is very specific).
*Qualifications for Prosthetics (dentures/partial dentures): All clients who qualify for preventive and restorative dental treatment will obtain prosthetics if needed. Clients who are low income that do not meet our regular criteria listed above, that need prosthetics only.
DOCUMENTS REQUIRED
Must provide proof of income. A benefit verification letter for a Social Security check, an awards letter for food stamps or pay stubs. A formal letter from a case manager can be used as proof of income if approved by the EODDS Intake Coordinator. A letter from a medical provider may be requested if deemed necessary by an EODDS staff member.
