Mon-Fri 8:30am-5:30pm, Sat 9am-1pm; **1st Thu of each month 12 noon-5:30pm
INTAKE PROCEDURE
Appointment preferred; Walk-ins accepted on first come, first served basis.
FEES
None.
ELIGIBILITY
Must meet income restrictions; reside in service area, and be a woman who is pregnant, breastfeeding or has recently given birth OR an infant OR a child 5 years of age or younger.
DOCUMENTS REQUIRED
Must provide photo identification card(s), proof of income, proof of address and Social Security Card(s).
