Provides Baby Boxes containing either emergency formula or 15 diapers. EIS helps families up to four times a year. Visits must be at least two weeks apart.

ELIGIBILITY
Must be in an emergency or crisis situation and have a child 5 years of age or younger.
DOCUMENTS REQUIRED
Must provide photo ID/driver's license and ID for each child needing assistance. This can be a birth certificate, hospital footprints, vaccine/medical records, or placement/guardianship papers but not Social Security cards.
